Program Checklist for Self-Study
A complete self-study must include a checklist for the program files within the document. The program must identify the page number for all items on the checklist. The checklist aids the program in ensuring all required items are included in the self-study and is used by the review team and site evaluators for note-taking as they assess the self-study.
For full functionality of the forms downloaded from this page you must have the latest version of Adobe Acrobat Reader. This software is free and can be downloaded from the Adobe website by clicking on the link provided. For the ability to merge documents of any format into a single PDF file, which is necessary for self-study creation, the full version of Adobe Acrobat is necessary.
