Self-Study Forms
Self-study refers to the process of program self-assessment and the document the program writes to report on the process and outcomes of its self-assessment. A self-study submitted to the JRCNMT for initial or continued accreditation must contain specific forms and documents and follow a structured format. Refer to JRCNMT Self-Study Instructions for details. All the necessary forms are provided below.

Program Volume of Self-Study
Affiliate Volume of Self-Study
Refer to the Self-Study Instructions for a list of all information that must be included for each affiliate along with sequencing of the documents. There is a column on the instruction sheet for each type of affiliate (major, minor and academic).
Checklists for Self-Study
A complete self-study must include a checklist for the program volume and one checklist for each affiliate. There are unique checklists for the various types of affiliates - major, minor and academic. The program must identify the page number for all items on each checklist. Checklists aid the program in ensuring all required items are included in the self-study and are used by the review and site visit teams for note-taking as they make their way through the self-study.
