Use this form to notify the JRCNMT of personnel changes at the program, sponsoring institution or an affiliate. The JRCNMT Resume' Form (below) plus supporting documentation must be submitted with the change form. Programs have 30 days from the staffing change or vacancy to report it to the JRCNMT. Use this form to notify the JRCNMT of the following: changes in program contact information; change of the PD's immediate supervisor or institutional President/CEO; name and address changes of affiliates; deletion of an affiliate; or to request a student capacity change at an affiliate. New programs offered by distance education and accredited programs adding a distance education track or converting the existing program to distance education delivery must submit this completed form. For existing programs this application must be submitted at least 6 months in advance of the proposed change in curriculum delivery.
