Affiliate Application Forms
Standard F1.3 requires programs submit an application for all academic and clinical facilities utilized by the program to comply with JRCNMT accreditation standards. Please review the Affiliate Application Instructions before preparing a submission.
An accredited program may submit an application to add new major, minor or academic affiliates at any time between regular reaccreditation processes. Submissions outside the cycle require an application fee while including the applications in the next regular self-study, if the program can wait to use the site(s), does not require an application fee.
